Supported living for service users with complex needs

Stockport Council are conducting some preliminary market intelligence gathering to ascertain if there are any providers able to meet the complex needs of the two individuals which are described in the attached brief. The provision is ideally required within the Borough of Stockport however consideration will be given to providers operating slightly out of Borough. Providers are asked to submit a summary statement of no more than 500 words detailing how they can meet the requirements outlined in the attached brief and demonstrate their experience to date of delivering these services. Details of what the provider should complete are detailed in the brief and are below for information. The provider will be required to complete: - Thorough assessments, care and support plans - Risk assessments - Positive behavioural management plans – techniques to modify poor / risky behaviour. - Social inclusion / activity plans – meaningful activities. Providers should note that this submission is for information only and will not in any way be scored. This is an information gathering exercise only.
