NHS England (North) Yorkshire and Humber are accountable for the provision of local GP practice services in the North Yorkshire and Humber area. Under the co-commissioning arrangements CCGs have delegated responsibility and together with NHS England wish to procure the following practice in the North Lincolnshire area. NHS England (North) Yorkshire and Humber is proposing to commission an APMS contract which is currently delivered from the Weelsby View Health Centre, Ladysmith Road, Grimsby, North East Lincolnshire, DN32 9SW. It will be a requirement that the services are offered from these premises. The aim of the service is to deliver essential, additional and enhanced services to the registered patients of the practice. The commencement date for the practice will be 10.8.2016. NHS England (North) Yorkshire, and Humber is proposing to commission an APMS contract which is currently delivered from the Weelsby View Health Centre, Ladysmith Road, Grimsby, DN32 9SW. The aim of the service is to deliver essential, additional and enhanced services to the registered patients of the practice. Core Service Payment for the registered service is 85,13 GBP per registered patient and the weighted list size as of 1.1.2016 was 3 902 patients. The delivery of the Quality and Outcomes Framework and delivery of Directed Enhanced Services will be paid in-line with national specifications and finances (and in addition to the remuneration of the APMS contract). It is proposed that the T and C of service, including tariff arrangements, will reflect those contained in Alternative Provider Medical Services (APMS) contracts.
