Tender description

Phase 2 of the pan-London NHS continuing healthcare domiciliary care AQP is open for applications until 10 June 2016 at https://procontract.due-north.com/register. Successful providers will be added to a domiciliary care AQP list which is used by NHS Clinical Commissioning Groups (CCGs) and discharge teams to make continuing healthcare placements. The AQP will be the standard route of commissioning for the participating CCGs. This means that domiciliary care packages will be commissioned with AQP providers. Only in exceptional circumstances will care packages be commissioned with non-AQP providers. Please see the attached domiciliary care AQP information pack for further details.
