Tender description

LCPH are commissioning a revised service for Health Trainers ( Lot 1) in community and primary care settings for adults who with interventions covering, weight,exercise,sensible drinking,social isolation,stress and anxiety could improve their health & wellbeing. The Community Health Ambassadors ( Lot 2) will also operate in their communities in which they live and engage with the local people in order to effectively deliver health promotion information and the promotion of engagement with Pimary Care as an initial point of access to other health services.
