Tender description

NHS West Norfolk Clinical Commissioning Group (the Commissioner) intends to integrate the local provision of Care Home Matron services following the adoption of a Most Capable Provider approach within its current provider base. This integrated service model will ensure the best outcomes for West Norfolk patients residing within a care home setting by providing an acute visiting service aligned to GP practices along with a proactive approach to medication reviews and optimisation and the development of patient centred care plans that will lead to a reduction in avoidable admissions to an acute setting. No expressions of interest are sought from the publication of this advisory notice. This Prior Information Notice (PIN) is to advise the marketplace of Commissioners' intention to undertake a service acquisition via the Most Capable Provider (MCP) Process under the Procurement, Patient Choice and Competition Regulations (2) 2013 (PPCC Regs).
