Tender description
The Wellbeing and Independence Network (WIN) will incorporate a range of practical, social and emotional support which promotes and enables people's ability to live well, independently and with a good quality of life. Within this will be some services which are explicitly funded (with some co-funding from NHS Stockport CCG) and commissioned by SMBC Adult Social Care and others which will support and develop social action within the community. The 4 ‘Lots’ or schedules making up the network are: Schedule A — Well-being and independence at home; Schedule B — Independent living and well-being in the Community; Schedule C — Well-being and independence through Community transport; Schedule D — Well-being and independence through social action. These are all detailed separately in Part 3 of the Service Specification. However they will all have some common features and be required to work collaboratively as part of 1 network to serve the best interests of service users and to meet emerging needs. Lot A — Annual Budgets Lot A — Well-being and independence at home — 300 000 GBP; Lot B — Independent living and well-being in the Community — 300 000 GBP; Lot C — Well-being and independence through Community transport — 120 000 GBP; Lot D — Well-being and independence through social action — 150 000 GBP. The guideline financial envelope will be a maximum Annual Contract Value for each schedule as listed above. The contract term is expected to be 3 years with the possibility of an extension of up to 2 further years. Within this financial envelope the bidders will be asked to quote the total hours which they will be able to deliver, inclusive of all aspects of preparation, travel, consultations, support, attendance at meetings, administration, etc. and showing how this will be supplemented by any other funding or revenues as part of the model.
