Tender description
Rochdale Adult Care ,NHS Heywood, Middleton and Rochdale CCG and partners are committed to improve the community provision for People and families facing Dementia. At the same time, we have been reviewing some of the demographic changes for clients with Learning Disabilities as they grow older and live longer lives and listening to feedback from families dealing with Dementia. From this we are delighted to offer the market an exciting opportunity to work with us on a holistic service for people with Dementia and their carers and also a residential offer for people with Learning Disabilities who are living longer lives Rochdale Adult Care and the CCG are keen to see proposals which utilise creative approaches in service delivery and development. Of particular interest to us are offers which align to National Guidance and Strategies for Dementia and support the vision for making Greater Manchester the best place in the world for Dementia Care. Springhill Resource Centre is a Council owned building which provides an excellent opportunity for refurbishment. The building is large, light and spacious and mainly on one level. It benefits from attractive outdoor space, parking facilities, good access and is in a desirable location, with older persons population within the immediate vicinity. The Council and the CCG have had significant refurbishment investment approved, which aims to utilise best practice in Dementia design wherever possible. The newly refurbished building will be a valuable asset for local Health and social care provision and a key part of our vision for holistic, person centred services. As part of this provision for the future, the Council is offering the lease of the building on a very reasonable lease of 25 000 GBP pa. The CCG have agreed to make a contribution to the provider towards the lease/running costs of the building, highlighting or shared commitment to the sustainability of delivery. We envisage that this will become a living well dementia hub, with strong connections with the community, offering opportunities to socialise, build networks and access high quality co-ordinated services. We are keen to provide opportunity for the development of a multi-purpose dementia service that meets a number of agendas: — An ageing Learning disabilities population with increasing prevalence of dementia who need to be able to access a residential care offer that meets their needs — Increasing dementia in the population as a whole and a need to keep people with a dementia diagnosis living independently for as long as possible — by providing day support, meaningful activities, building social/ community connections, providing respite for carers that meet the needs of the service user and the carer. — The need to support people who are diagnosed with dementia to understand their illness and provide information and advice about their prognosis and coping strategies for managing their illness — The need to support the carers of people with a dementia diagnosis to help them to understand the illness, providing information and advice and support with coping strategies for managing the changing and sometimes challenging behaviours of their relatives. The Service are looking for a Lead Provider or Consortia to deliver all services in the specifications.
