Sheffield CCG - Mental Health Liaison Service

Tender description

The service will be a Comprehensive Mental Health Liaison Service (hereafter referred to as MHLS). In operating as a Comprehensive service model, in addition to offering direct interventions, it will also offer packages of education tailored to assist hospital, primary care and community staff skilled in the provision of physical health services to also meet the mental health needs of patients. The direct intervention service will provide face to face patient care in STHFT Emergency Department (ED), hospital wards and outpatient departments (OPD) and will also support care outside of those settings, and in primary care to enable optimal management of patients leaving or being discharged from hospital. The service will be integral to promoting the SCCG ‘Care outside hospital strategy’ and will work with hospital practitioners and General Practitioners to support the implementation of the Clinical Assessment, Service, Education, Support (CASES) and Active Support & Recovery (ASR) service models within Elective Care. It will support delivery of the recommendations within the Mental Health Taskforce report, particularly with regards to the planned pathway guidance, and will be expected to achieve the latest nationally mandated access standards.
