We are seeking support in shared accommodation for a male aged between 45-50, with complex needs in the East Hampshire or West Sussex area. PLEASE NOTE: If you submit a proposal, we expect you to include your weekly cost on the response form. Without completing this section, your submission will not be shortlisted and considered by the support worker. Gloucestershire County Council on behalf of 2gether NHS Foundation seeks to commission a package of care and support for an adult with Disabilities who lives in Gloucestershire. Responses to this Further Competition should be made via the Supplying the South West system by registering an interest against this advert. Once this step is completed you will receive an email containing the link to further information. Responses should be made using the blank pro-forma within that information. PLEASE NOTE: Any package purchased will be made under the Care & Support Spot Contract attached to this Further competition. Upon agreement of any package the Purchaser shall issue an Individual Service Contract in respect of the individual (part of the Framework Contract attached) which will be duly signed by the Provider and the Purchaser.
