Tender description

NHS Heywood, Middleton and Rochdale Clinical Commissioning Group (NHS HMR CCG) are inviting suitably qualified and experienced Providers to submit Pre-Qualification Questionnaire documents and, if invited, tenders for The Provision of an Anticoagulation Therapy Monitoring Service. The service aims and objectives are to: • Deliver a high quality, responsive, cost effective anticoagulation therapy initiation, stabilisation and on-going monitoring service, for patients aged 18 and over, who require short-term or long-term (often lifelong) prescription of a Vitamin K antagonist (e.g. warfarin, sinthrome). • Prevent stroke or uncontrolled bleeding through effective anticoagulation control. • Deliver anticoagulation therapy prescribing and monitoring in accessible community settings i.e. close to patient’s homes, or in patients’ homes where the patient is housebound or has short-term mobility problems. • Deliver lean and clinically-evidenced treatment pathway for patients • Encouraging patients to self-manage their coagulation and treatment compliance Providers will be expected to demonstrate engagement and involvement of the following groups in the delivery of this service: • CCG – medicines optimisation, commissioning, primary care improvement • GPs • Pharmacy • Haematology and other relevant clinical outpatient specialities. NHS HMR CCG are looking to appoint two Providers to provide the Anti- Coagulation Therapy Monitoring Service. The indicative maximum annual budget available for this service will be £679,227.00. The tender process will be conducted similar to that of a two stage restricted procedure. The contract duration will be for 5 years.
