Tender description

To commission a transport service that operates a dedicated discharge crew to facilitate same day discharge and where appropriate, a transfer and A&E take home service, operating between the hours requested by the acute trust and within the commissioned hours, however the service must be responsive to fluctuations in demand, both from day-to-day and a longer term basis. To commission a service that will support the discharge of admitted patients at up to 1 of the 3 sites within South Yorkshire, to their home address or stated discharge address if staying with family or carer, including patients whose permanent residence is at a residential or nursing home.
