Tender description
This contract is for preventative, non-statutory services currently provided by the voluntary sector. It is for support for both carers and people with health conditions or disabilities and is intended to deliver four primary outcomes that underpin health and wellbeing. These outcomes are: • People can live independently in their own home • People are able to participate in their community • People are not socially isolated or lonely • People are safe and secure
