Tender description

Citizen-Driven Health is a citizen-driven, asset-based, whole community initiative, in support of the NHS Wales preventative agenda, to both improve the health and wellness of older people and reduce the cost of care to the NHS and statutory partners. The initiative has been developed by Cardiff and Vale University Local Health Board and Cardiff Council and is supported by the Welsh Government who have provided funding from January 2016 for a 9 month period.
