Tender description

NHS Bassetlaw Clinical Commissioning Group (CCG) is seeking providers of services for adults requiring domiciliary care in their own homes who are eligible for NHS continuing healthcare funding. This primarily includes those people who are eligible for continuing healthcare and have been assessed on the Fast Track, as they are believed to be close to the end of their life. The Fast Track is reserved for people with a rapidly deteriorating condition that may be entering a terminal phase.
