Tender description
The aim of the Home to Hospital Discharge Service is to assist in the smooth transition of individuals registered with a Nottingham City General Practice, from hospital to home following discharge from acute care, provided across the QMC and City Hospital campuses of Nottingham University Hospitals (NUH). Enabling individuals to retain their independence through support to return to and remain in their own home ensuring that individuals are not readmitted when it is not medically necessary and do not remain in hospital longer than they need once their medical and nursing requirements have been met. The provider will aim to support individuals within their own home, providing a service which is timely, responsive and reliable to meet the changing needs of individuals regardless of diagnosis, irrespective of gender, race, religion, ethnicity and physical or mental ability. The objectives of the service are: — To be a domiciliary support service, not a nursing or personal care service; — To support adults (18 years +) discharged from acute care with the daily task of living; — To support individuals resident in Nottingham City and registered with a General Practitioner within Nottingham City; — To provide and/or signpost holistic information to support the individuals independence; — To reduce unnecessary stress and strain to the individual and their carers; — Prevent unnecessary increased lengths of stay contributing to a reduction in dependence on long-term care; — To support individuals to be cared for in their own home actively avoiding unnecessary readmissions.
