• In Extra Care Housing Schemes, people who have been identified as having a care need (the majority of whom will meet the eligibility criteria for care services), live within their own self-contained flats, exclusive of board, but with the opportunity to purchase a meal or meals on site. Appropriate support, including personal and social care and support provision is provided by staff employed by the care provider, in accordance with an on-going assessment of need. This domiciliary care is provided with a reablement focus (where appropriate), to each individual in their own home, by promoting their independence and well-being. • 24 hour background support is available in each scheme to all residents (Band 1). In addition to the 24 hour support some residents also receive additional based on the level of support received by residents each week from the care provider (Bands 2 – 5). • Wirral Council seeks to commission a Domiciliary Care Service to operate 24 hours a day, 365 days a year, delivering both planned and unplanned care to residents within Wirral’s Extra Care Housing schemes. • The service will operate for two full financial years with a proposed start date of 14th June 2016 to 13th June 2018, with an option to extend to 13th June 2019, and a further option to extend to 13th June 2020. • We seek to deliver the service with a provider who is already accredited to the Wirral approved provider list. • The service will cover the whole of Wirral, and is advertised as one single lot to provide support in all current and future extra care schemes.
