Rapid Response Domiciliary Care

Tender description

Cheshire East Council is inviting applications from providers of domiciliary care to deliver a service that will complement our existing service providers, through delivery of a Rapid Response Service, with an enabling approach, to prevent unnecessary hospital admissions and support the prompt discharge of patients from hospital back to their homes and avoid unnecessary longer term care packages where appropriate. This contract will be for a period of 12 months or until the available funding expires. The service will provide domiciliary carers deployed at short notice no later than 6 hours after referral for hospital discharges and no later than 2 hours for hospital avoidance, to support people for a maximum of 4 weeks to avoid admission, or for a maximum of 2 weeks to support their timely and safe discharge from local hospitals.
