AQP for Hampshire Domiciliary Care and Supported Living Services

Tender description

The Commissioner intends to commission support to individuals in community settings that is based on the principles of enablement, through outcomes-based contracts, under an AQP model. The Hampshire Clinical Commissioning Groups will achieve this through partnership arrangements with service providers that embrace an outcomes based, personalised approach to support. This is a developmental specification. Service providers will accept and acknowledge that aspects of this specification may change. The Service will be co-produced, personalised and outcomes focused, providing support predominantly to service users with Learning Disabilities and Autism Spectrum Conditions, and also to service users with Physical Disabilities, Sensory Impairments, Mental Health Problems, a Forensic History and other Vulnerable Adults (16 years or older) to include (but not limited to) the following service models: • Support to individuals which may include the provision of personal care • Shared support to two or more individuals which may include the provision of personal care • Overnight support to individuals or groups which may include the provision of personal care • Community Support Networks • Support to access employment, education or other vocational activities • Landlord Supported Housing
