Tender description

This is a Prior Information Notice (PIN) and Request for Information (RFI) published on behalf of NHS Liverpool Clinical Commissioning Group (NHS Liverpool CCG). The purpose of this PIN/RFI is to gauge market interest and conditions for The Provision of Comprehensive Care to meet the needs of those at End of Life Service. NHS Liverpool CCG is seeking expressions of interest from suitably qualified service providers to deliver a comprehensive care and support service for people (and their families) clinically determined as being in the last 12 weeks of life. Care will be provided in the persons preferred place of care such as home, hostel, care home etc. The first national end of life care strategy was produced in 2008 and generated significant momentum and energy leading to many improvements in end of life care. We aim to transform health care by ensuring that services are person centred, responding to individual need and by providing the very best care services now and in the future. High quality, person/family centred care for people at the end of life care has been identified as an essential priority as part of the Healthy Liverpool strategic plan. This service is currently being provided in the city to provide clinical assessment, care provision and support as detailed below.
