Waltham Forest CCG Anticoagulation Services Community Pilot

Tender description

One prime provider will manage all aspects of community anticoagulant provision for Point of Care testing (POCT) and monitoring of stable patient INR across Waltham Forest via the use of hubs, which include GP Practices/ Community Clinics and Pharmacies. The services will provide on-going management of warfarin treatment taking appropriate blood samples, testing and providing dosing advice to patients, development and updating of written treatment plans. The service will include domiciliary visits to housebound patients for testing and management. The anticoagulation service will have responsibility for setting up call and recall systems, schedule appointments, as well as maintaining accurate care records for their patients using the Computer Dosing Support Software (CDSS) to support this.
