Shared Lives Service

Tender description

Staffordshire County Council wishes to invite tenders for the provision of a Shared Lives Service. The Shared Lives Service matches eligible Customers with a compatible and approved Shared Lives Carer, leading to the sharing of family and community life in the form of regular visits or long term accommodation. The purpose of the Shared Lives Service is to support eligible Customers to live as independently as possible and to achieve or maintain their desired potential in relation to their physical, emotional, intellectual and social capacity with the aim of reducing dependence and promoting greater independence. The Service is accessed by a range of eligible Customers aged 18+, including people with learning disabilities and Autistic Spectrum Conditions, Older People, People with Mental Health Needs, those with a Physical or Sensory Impairment, People with Complex Needs plus referrals of Younger People in transition into Adult Services. The main components of the Scheme are as follows: (1) The management, promotion and development of the Service by the Provider (2) The recruitment of New Shared Lives Carers (3) The provision of support and training to new and existing Shared Lives Carers (4) The provision of support to Customers in relation to long term, respite, Day or evening sessions (5) The matching of Customers and Shared Lives Carers, the placement of Customers with Shared Lives Carers and the monitoring of these Placements
