Tender description

The aim is to provide a comprehensive community anticoagulation service which will serve patients, aged 16 and over, registered to a GP practice within the London Boroughs of Bexley who require: •Initiation of Warfarin Therapy OR non vitamin K antagonist New Oral Anticoagulant (NOAC) •On-going monitoring of INR for the purposes of ensuring oral warfarin (or other vitamin K oral anticoagulant) dose remains therapeutic – at least delivery of 65% time in therapeutic range for individual patients on vitamin K oral anticoagulants.
