Tender description
Medway Council, in partnership with NHS Medway Clinical Commissioning Group (CCG), is seeking to establish an Integrated Community Equipment Service for the people of Medway. The purpose of the service is to obtain, deliver and install and maintain community equipment on loan to people living in the community. We are looking for a provider that, working with partner organisations across health and social care, can deliver a safe, effective and personalised community equipment and minor adaptations service to support people to remain in their own homes. This will be a jointly commissioned service provided in accordance with the statutory responsibilities of Medway Council and NHS Medway CCG. It will be managed as part of the Better Care arrangements through a Section 75 Partnership Agreement. In line with demographic changes across the country, Medway is experiencing increased demand on its services with increasingly limited resources in both health and social care. Significant investment in preventative activities is required to enable people to live longer, healthier lives independently in their own homes. This requires a shift from a reactive to a proactive and preventative-focused health and social care system. Our plan for the next five years is a fundamental whole-systems approach resulting in improved quality and experience of health and social care users and improved individual outcomes. In line with legislation, service users and patients across the health and social care sector of all ages will access these services through various health and social care routes.
