Tender description
NHS Heywood, Middleton and Rochdale Clinical Commissioning Group (NHS HMR CCG) are inviting suitably qualified and experienced Providers to submit tenders for The Provision of Integrated Care Provider Collaborative for Integrated Neighbourhood Teams (INTs). INTs are multidisciplinary teams of health and social care professionals who will be aligned to GP practices across the borough. INTs is one of the major components of health transformation that will enable the move to successful out of hospital care. These multidisciplinary teams of health and social care professionals will work in an integrated way with GP practices across the borough to improve case management of those with long term conditions, aiming to reduce crises and enable patients to manage their own conditions more successfully. The INTs will serve the population aged 18 and over (16 and over for patients with a diagnosis of epilepsy) registered with a Heywood Middleton and Rochdale GP practice. The INTs will also serve the registered population of Whitworth Medical Centre which is a member practice of NHS East Lancashire CCG. The focus of INTs is to streamline case management of patients with multiple long term conditions through multidisciplinary working both on a daily basis and through more formal monthly case review meetings.The Integrated Care Provider Collaborative will develop the relationships and pathways that will ensure such collaborative working functions to the benefit of patients. This new approach will reduce the number of patients needing hospital care. The INTs will also work with patients so that they can share in decisions about their care. Within the team it should be a given that professionals can cross refer to another member of INTs and the wider services across the localities. INTs will support the transformation of services to support the NHS and local authorities prevent duplication and use their resources more effectively, while at the same time improving the care for patients. Delivering a person-centred and integrated approach to the provision of care for adults is a key priority for health and social care in the Rochdale Borough. This type of care delivery requires a fully integrated response across health and social care linking to health and well-being services, housing, employment, benefits and voluntary sector as and when appropriate. Commissioners are keen to look for innovative models of care delivery and providers that are willing to change models to improve care as needed based on robust evidence and clinical leadership.
