Tender description

The aim of the Newham Community Prescription is to increase physical activity/healthy behaviours across the borough of Newham through delivering an evidence based service for Newham residents, over 18 years of age who meet the eligible criteria: at risk of Diabetes and/or at risk of Cardiovascular Disease, with a BMI >=30). The ‘gateway’ provider will act as the prime provider organisation, with the expectation that at least 50% of the activity interventions being offered will be delivered by local community 3rd sector organisations , excluding the gateway provider. The service will comprise of two components: a) Activity based interventions, covering up to 24 weeks, comprising, 1 hour per week of moderate intensity physical activity. Throughout the intervention programme, service users should be supported to achieve the recommended 150 minutes per week of moderate intensity activities. b) An outcomes based incentive will be awarded after 24 weeks, based on a service user achieving the physical fitness KPI at week 24.
