Cardiac rehabilitation service for North East Hampshire and Farnham area

Tender description

The North East Hampshire and Farnham Clinical Commissioning Group works with local hospitals, specialist health providers, GP’s and other family health practitioners, local authorities, voluntary organisations as a whole to make sure services are doing the right things and are being provided from the right places to support the health needs of the local population. The North East Hampshire and Farnham area covers the towns of Fleet, Aldershot, Yatelely, Farnborough and Farnham. There are currently 24 GP practices providing care to a combined registered population of 220,000 patients. Annual budget for this service is expected to be in the region of £170,000. During 2014/15 approximately 395 patients being offered cardiac rehabilitation locally. The current process requires providers to complete a Request for Information to guage the interest in the potental service, the documents can be access through the InTend portal.
