15 105 Specialist Domiciliary Care and Support in a Supported Living Setting for an Adult with Autism in Crewe/Partington

Cheshire East Borough Council is required to provide 24 hour care in an individual's own home and various community settings. The individual is an 18 year old male with physical and learning disabilities. He requires constant support and supervision including overnight care which will be provided via a combination of individual 1:1 support and shared care. A supported living setting in, or within a 5 mile radius of the Partington area <b><i><u>OR</u></i></b> in, or within a 5 mile radius of the Crewe area, sharing with between 1 and three other adults of similar age and ability is required to meet the individual's assessed social care needs. Support staff will need to be skilled in working with adults with autism, complex learning and physical disabilities. The individual attends school in Crewe, Cheshire which will continue until July 2016. The school year commences 3 rd September 2015 and finishes on 22 nd July 2016 (please note that dates may be subject to change at the school's discretion). Transport is funded by education and will be provided by an external transport provider
