Any Qualified Provider (AQP) Community Packages of care for Children and Young People with Continuing Care Needs – Nene CCG, Corby CCG and Milton Keynes CCG

NHS Nene, Corby and Milton Keynes Clinical Commissioning Groups (CCGs) are seeking to establish a list of Approved Providers who can provide packages of care in the community to children and young people who meet the threshold for continuing care (CC) support in accordance with the specification. This would include children and young people with complex physical disabilities; those requiring long term ventilation; those with life limiting and life threatening conditions; and those with acquired brain injury The purpose of the care package is to support parents/carers to care for their child at home within the context of their family whilst living as independently as possible. This procurement main aims are: • To establish a list of Approved Providers, who can deliver high quality, outcome focussed packages of care for children and young people living at home with their families. • To develop a service which can meet the needs of children, young people and their families in Northamptonshire and Milton Keynes in accordance with the principles of the National Framework for Children and Young People’s Continuing Care guidance and the national personalisation agenda. Please note that under ‘Any Qualified Provider’ contracts there will be no guarantees of volume in the contracts awarded. Further, prices (e.g. hourly rates) are set.
