Sutton CCG Intermediate Care Beds Contract

NHS Sutton CCG is seeking nursing home Providers to provide a total of 22 Intermediate Care Bed to Sutton CCG within the London borough of Sutton. Sutton CCG is seeking two or three Providers and each will provide a minimum of 5 Intermediate Care Beds. A maximum of 11 Intermediate Care Beds will be awarded per Provider. The Providers will work with the CCG and the Intermediate Care Community service (part of Sutton Community Health Service) to provide nursing care beds for individuals who require 24 hour supervision during their rehabilitation programme. They will be part of the wider Intermediate Care Service that strives to deliver a service that is patient focused and delivers on prevention of admission, clinical effectiveness, outcomes, and supports independence. This service will provide a responsive Intermediate Care Service to: • facilitate hospital discharge; and • prevent admission to acute settings from the A & E department and the community. The Intermediate Care Bed Providers will be a key link in providing flexible rapid assessment and care delivery post hospital discharge or to prevent a hospital admission, to meet a range of nursing care needs. The individuals accessing these beds will have nursing and therapy needs. The therapy management will be provided by Sutton Community Health and the nursing home staff will be responsible for the assessment, planning and delivery of nursing care, and support the delivery of the therapy interventions by following the therapy care plan written and agreed by Sutton Community Health.
