Tender description

In a literature review of the benefits of welfare rights, advice that looked at provision within primary care it was highlighted that welfare advice, environment, socio-economic status, health and quality of life are all factors that have complex, multifaceted interdependencies. The review showed that there is a growing body of studies that suggest welfare rights advice, through improving take-up of entitlements, has a positive impact on health and social wellbeing, and that placing advisory services in a primary care context is particularly effective for reaching eligible non-recipients (Wiggan & Talbot, 2006). The aim of the service is to positively impact on the social determinants of health by providing access to a range of social welfare advice services which meet the needs of patients; maximising income and improving health and well-being. The service will provide expertise and supportive advice for people who may have temporary or longer term physical and/or mental health conditions. The service will; assess needs and implement goal-focussed action plans with the aim of maximising income for the individual and improving overall health and wellbeing. This is a three years contract (£599,685- activity levels and outcomes achieved) with two years possible extension (with total five years contract value of £999,475- activity levels and outcomes achieved). The awarded Provider (prime contractor) can choose to allocate specific roles & responsibilities to other providers (i.e. sub-contractors) but will be responsible/ held accountable for the successful delivery of the entire service.
