This is a Contract Award Notice. This service will deliver low to high intensity non-accommodation based care and housing related support and will meet the individually assessed needs of the people who will receive the service, as reflected through their outcome based person-centred support plans, and in line with the expectations of the "Community Services Directorate Plan" and the "Valuing People" White Paper. This service will also deliver low to medium accommodation based housing related care and support and will meet the individually assessed needs of the people who will receive the service, as reflected through their person-centred support plans, and in line with the expectations of "Local Services for Local People" and the "Valuing People Now" White Paper. The service will deliver the specification within the geographical boundaries of West Berkshire District Council
