The Commissioners jointly commissioning the Service are:• Wirral Council.• NHS Wirral Clinical Commissioning Group The Lead Commissioner/Contractor for this procurement is Wirral Council (WC).The Contract will run initially for 4 years starting on 1 JUNE 2015 with options to extend for up to 3 years and a further 2 years (4+3+2) subject to: Continued funding, Continuing need, the achievement of specified outcomes and the performance of the ProviderThe commission of these services forms part of the Better Care Fund (BCF) programme running from April2014 – March 2016.  The BCF is explicitly intended to integrate Health and social Care systems at a local level.  Vision 2018 is the local application of the BCF and sets out the Integration objectives for the future.The commission also links closely to the Council’s Commissioning and Procurement Boards, and transformational change within Wirral Council Commissioning.
