Tender description

Lewisham Council on behalf of Lewisham Clinical Commissioning Group are looking to procure a Primary Care Dietetics Service for adults in Lewisham. Lewisham seek to commission an experienced and suitably qualified organisation to deliver a primary care dietetic service within Lewisham. The commissioned service will consist of the following three interlinked components delivered by one provider: 1. Continuation of an existing service that comprises of: a) practice-based dietetic service with a focus on clinical conditions requiring specialist dietetic intervention, delivered in all 41 GP practices in Lewisham. b) borough-wide specialist dietetic weight management service as part of the community tiered adult weight management pathway. 2. A new dietetic provision to Nursing Homes in Lewisham This will provide a service to nursing homes including Learning Disability and Mental Health care homes to help reduce the incidence of malnutrition. The dietetic intervention will include training and individual client assessment. 3. A new dietetic provision for patients discharged on oral nutrition support from secondary care This element of the service will work closely with the existing Lewisham Integrated Medicines Optimisation Service (LIMOS) team. The service will oversee, monitor and review oral nutrition supplement prescriptions. The service will also develop and implement a malnutrition treatment pathway. The term of the contract will be for 3 years, with the option to extend the contract for a further 2 years at the discretion of Lewisham CCG. The upper budget limit for this project is £490,000 per annum. There will be an additional CQUIN payment, which is detailed in the payment schedule of the contract. This procurement will be undertaken by Lewisham Council under the Light Touch Regime under Regulations 714-73 and Schedule 3 PCR 15. This tender is being procured by Lewisham Council for and on behalf of the NHS Lewisham Clinical Commissioning Group, an entity wholly separate from the Council. Any contract which is entered into as a result of the tender will be solely with Lewisham Clinical Commissioning Group and not with Lewisham Council.
