KMC-AS085 CONTRACT for Kirklees Stroke Information, Support & Targeted Prevention; Six month Reviews following Hospital Discharge; and Communication Support

Kirklees Council wishes to appoint suitably qualified and experienced Service Provider(s) in respect of the Kirklees Stroke Information, Support and Targeted Prevention; Six month Reviews following Hospital Discharge; and Communication Support Service. The Services are required to be provided under each Contract for a period of 3 years from 1 October 2015 with the possibility of an extension for a further 1 year. The overall aim of the service is to achieve the best possible outcomes and quality of life for people affected by stroke, and their carers, and target prevention support to reduce the prevalence of stroke across Kirklees therefore the demand for high cost health and social care services. The Contract is made up of the following three lots with total contract value of up to £130,000 per annum and is broken down as follows: Lot A: Information, advice & 1-1 targeted support -£60,000 Lot B: Six Month Reviews - £49,000.
