Tender description

The NHS Central London Clinical Commissioning Group (the CCG) is looking for non-clinical in-reach workers, based in each of the London hospitals that serve the CCG population, to identify homeless patients at the point of admission. The in-reach workers will likely be attached to clinical teams at the 2 homeless GP Practices (Great Chapel Street Clinical Centre & Dr Hickey’s Surgery) for supervision. They will need to work closely with hospital & GP colleagues to determine patient eligibility for the Intermediate Care service the CCG will be providing. For those patients not requiring recuperation or further clinical input, the in-reach team would then need to help facilitate a suitable move-on plan for the individuals, with a view to managing early discharge from hospital and preventing the need for re-admission. A dedicated, skilled individual, with an understanding of the challenges that homeless patients experience, is required to organise homeless case conferences, intermediate care beds etc. on behalf of the two dedicated homeless GP practices in Central London, with a view to achieving positive patient outcomes. It is the complexity of both the group & the services that makes the flexible and tailored service necessary. Training to GP, hospital & hostel colleagues on homeless services will also need to be provided to ensure sustainability. Organisations which express an interest will be invited to attend a Suppliers Information/Market engagement event on 29th May from 9.30-12:00 in Central London. A maximum of 2 people from each organisation will be able to attend. The event is aimed at being mutually beneficial, helping inform the CCG with their procurement planning as well as allowing bidders to get a better understanding of the requirement/services, with an opportunity to input suggestions & share ideas. Please find attached a DRAFT service specification for the above services – this is likely to be revised to some extent before any actual procurement process. The contract value is circa £100,000 & the CCG currently intends that a formal procurement process will commence from the beginning of June 2015 through a notice on Contracts Finder. The CCG would like to see a contract awarded by the end of July 2015 & for services to commence ASAP thereafter. Note: An expression of interest is no indication of a commitment to participate in the subsequent procurement process nor does it infer any special status on those providers who do submit an interest. Neither will this notice or any organisation’s expression of interest commit the CCG to undertake any procurement of the Homeless Care Navigation and Hospital In-Reach services & the CCG has a right to withdraw the notice.
