Contract award notice for Dementia Advisor Service on behalf of NHS Eastbourne Hailsham Seaford, NHS Hastings and Rother, NHS High Weald Lewes Havens CCG. To support the individual CCG’s objectives, the contract duration will vary as follows: Hasting & Rother CCG 3 years, Eastbourne Hailsham & Seaford CCG 3 years, High Weald Lewes & Havens CCG 1 year. The service objectives are to collate, keep updated and disseminate information relevant to the needs of those with a suspected or diagnosed dementia and those that care for them in East Sussex. To provide personalised accessible signposting and liaison support for those with a suspected or diagnosed dementia and those that care for them. The service will be responsible for collecting and collating knowledge and information to facilitate access to and understanding of local health, social care and third sector/peer support available to people with a suspected or diagnosed dementia and their carers.
