TD0864 - The provision of Carers Services for the City of Derby

Tender description

Tenders are invited for the supply of Services in respect of Carers Services for the City of Derby.The carers support service will be provided to informal cares aged 18+. The requirements of the contract is to provide universal and preventative support to carers, targeted low level support to prevent carers breakdown and intervention support via a carers assessment and or emergency plan. Some carers who meet the eligibility criteria may be entitled to a personal budget will be to support the carer on maintaining their well-being and caring role for as long as they are willing and able.The current maximum level of funding for this service is 253,200 per annum. However, there may be scope for an increase in the annual funding of up to 280,000 as a result of the volume of carers assessments exceeding beyond a 5% growth over the previous contractual year.The initial contract period is 3 years with a potential to extend for 2 further 12 month periods. Total contract length is 5 years.The deadline for tender responses is 2pm on 8th January 2015.Please note, suppliers wishing to express interest and obtain the documentation, must do so by 15:00 on 7th January 2015.
