The SLAs consist of up to three service elements (dependent on individual GP sign up): — Integrated Sexual Health Provision in Primary Care — NHS Health Checks – General Practice — Locally Enhanced Service Shared Care for Substance Misuse Integrated Sexual Health Provision in Primary Care The service supports delivery against the three main sexual health Public Health Outcome Framework (PHOF) measures: —Reduce under 18 conceptions —Control Chlamydia by capturing positive diagnoses [15-24s] —Reduce the number of people presenting late with HIV at a late stage of infection Estimated annual value: 353 000 GBP NHS Health Checks – General Practice The aim of the service is to provide the NHS Health Checks Programme to eligible people aged 40-74 years (without evidence of CVD) in order to: —Enable more people to be identified at an earlier stage of vascular change, with a better chance of putting in place positive ways to reduce the risk of premature death or disability. —Enable the prevention and early detection of diabetes. —Offer a real opportunity to make a significant inroads into health inequalities, including socio-economic, ethnic and gender inequalities Estimated annual value: 168 750 GBP Locally Enhanced Service Shared Care for Substance Misuse The scope of this agreement is for GPs to provide substitute drug prescribing to stable clients within a shared care programme and care for all the general medical health care needs of these clients Estimated annual value: 40 000 GBP.
