Tender description

The Service Provider will provide a responsive and safe domiciliary care provision in the rural areas of West Berkshire to support packages of care, earlier discharge and preventable re-admissions from/to hospital utilising experienced care professionals, and covering all care needs (including dementia, physical disability etc). This is a pilot scheme for 12 months.
