Islington CCG Locality Navigators Service

Tender description

Islington CCG proposes to re tender for an enhanced navigator service which will directly support the new locality community service structure due to commence in September 2015. The service will provide greater capacity within the system to provide improved and more detailed signposting and case management services to patients identified by health and social care professionals as being in need. The expanded service will also support and develop new ways of working across the whole community service landscape as part of the overall Islington Integrated Care strategy.
