Step Up Assessment model pilot

Tender description

North Staffordshire CCG and Stoke on Trent CCG commission health services for around 492,000 residents are publishing this Pipeline Notice to inform interested providers of the intention to tender for a Step Up Assessment model pilot. It is anticipated that this will be a primary care medical service which will focus upon the triage and treatment, assessment, diagnosis and onward referral and care planning for a wide cohort of patients across 365 days per year. The service will be expected to develop close links with the community provider with regards to access to the step up bed base and support within the community through the utilisation of intermediate care, re-ablement, voluntary care, social care and mental health services. It is also expected that there will be senior level leadership and oversight from suitably qualified medical staff in order to assess and agree treatment and care plans, and provide ongoing monitoring as appropriate for those patients who are deemed suitable for transfer to the Intermediate Care Service or other community services. The provider must ensure all appropriate support and resources are in place, including nursing and therapeutic services and prescribing, to provide the appropriate level of care for patients within the Step Up Assessment model. Interested providers will therefore be expected to show examples of delivering a clinically effective, high quality service at scale, including demonstrating successful implementation and delivery. The CCG’s will be holding a bidder event on Wednesday 13th May 2015 from 12.30pm – 16.00pm in Newcastle Under Lyme where we would like to invite interested parties to come along to discuss potentials for service models and delivery within North Staffordshire in order to share a vision and proposal on the way forward, gather views and comments from a range of providers and interested parties and to jointly develop an model to empower a single provider to drive innovation and transformative change in care pathways. It must be stressed that this is purely a fact finding exercise and no tenders will be invited as part of this process. The responses received will help North Staffordshire CCG and Stoke on Trent CCG develop a service delivery model for the Step Up Assessment Centre. It will also help to refine the scope and specification of such a process. A formal tendering process may or may not commence at a later date.
