NHS Hammersmith and Fulham Clinical Commissioning Group (CCG) is inviting expressions of interest for the delivery of a primary care led dementia memory service, broadly based on the successfully implemented model at Gnosall Surgery in Staffordshire. THE DEADLINE FOR SUPPLIERS TO EXPRESS THEIR INTEREST, RETURN COMPLETED QUESTIONAIRES AND PROVIDE THE DETAILS OF EVENT ATTENDEES IS 12 NOON ON MONDAY 23RD MARCH 2015. *** The total value of the contract over three years is estimated to be in the region of £1.5m. The submission of an expression of interest is not a prerequisite to any subsequent procurement application.
