Our joint ambition is to improve health outcomes for people in Merton. As part of this, we want to strengthen the role of community services in improving the health of the community and thereby reduce the need for acute care. We want provision that is based on an integrated approach, with community services playing a leading role in enabling effective working between different professionals and organisations, so ensuring that people in Merton experience care that is seamless and well co-ordinated. We know that there will be many changes over the years ahead. Hence, we are seeking a Provider who will work flexibly, responsively, creatively and in partnership with commissioners and other partners; who will foster a “can do” attitude amongst staff and embed a culture of continual learning and development across the organisation. Putting people’s needs first and delivering improved outcomes for them must be at the core of all community service activities at all times.
