Carer Support Service Brighton and Hove PCT invites tenders from suitably qualified and experienced Providers to provide dedicated Carer Support Workers for the provision of carer support services across Brighton and Hove. To work alongside a new community service of multidisciplinary Integrated Primary Care Teams aligned to eleven groups of GP practices. These teams will support patients in the community with a wide range of long term and complex health care needs, improving communications, efficiency and patient outcomes and experience. The carer support service is aimed at providing carer support within these teams to carers of adults living in the community with the following aims: • Enable people with long term and complex health care needs to live independently at home by ensuring carers supported in caring role • Prevention of hospital admissions • Support for early supported discharge • Support and maintain hospital discharge • Support carers’ health and wellbeing • Maintain caring role This will be a new service to provide short term interventions including: • Carers assessments and Support Plans • Signposting/referrals to other support and provision of information • Communication with services on behalf of carers • Demonstration of basic caring tasks The aim is for procured services to operate from 1st April 2012. The length of the contract will be for 3 years. This procurement is being conducted as an “Open” procedure. Bidders are required to follow the instructions below and to obtain the documentation using the electronic process.
