Tender description

• The service must deliver on the Health Centre Core Criteria specified by the Delivering Equitable Access to Primary Medical Care programme (Gateway ref 9194) and the OOH National Quality Requirements. (www.dh.gov.uk/en/Publicationsandstatistics) • The registered list for the (8to8) service will focus on the vulnerable groups of patients defined as the homeless, asylum seekers and refugees and Travellers, to deliver significantly improved health outcomes. • The Health Centre through its services to unregistered patients will deflect unnecessary attendances at local A&E departments. It will also offer improved access to GP- led services and should complement local GP primary care provision. • Providers must demonstrate the ability to offer high quality, joined up care for Stockport patients and a value for money integrated service.
