Expressions of interest are being invited by Greater Manchester Commissioning Business Service (CBS) on behalf of Blackburn with Darwen PCT from all suitably qualified and experienced providers to provide a Healthy Lifestyles Programme on behalf of the Lancashire Condition Management Programme (CMP) hosted by NHS Blackburn with Darwen PCT The CMP is the NHS component of Pathways to Work, the Government initiative aimed at reducing the number of those claiming Incapacity Benefit. The aim of CMP is to support people with long term conditions such as mild to moderate mental health, musculo-skeletal or cardio-respiratory problems to manage their condition to lead a more productive life with a view to returning to work. The Healthy Lifestyles tender is being offered as 5 PCT areas. • Blackburn with Darwen • East Lancashire • Central Lancashire • North Lancashire • Blackpool Providers are able to bid for an individual PCT area or a number of areas and we also welcome consortium bids. Features of the service: • Provider to offer a flexible 12 week programme to clients and clients to attend a minimum of 1 session per week. • Physical activity can either be group or individual sessions. • Provider to act as either direct service provider and / or to act as a broker / coordinator. • Providers will, Support, Signpost and offer client’s physical activity solutions based on need and choice as well as educational advice. • Physical Activity solutions will include mobility, strength, balance, cardio-vascular endurance, co-ordination components. • Education advice will include barriers and benefits of physical exercise and activity, diet and nutrition, alcohol recommendations, smoking cessation, stress management and relaxation. Commencement date will be July 09 The overall approach of the healthy lifestyles module is to encourage individuals to become habitually more physically active and increase their awareness of a healthy lifestyle.
