Expressions of interest are being invited by Greater Manchester Commissioning Business Service (CBS) on behalf of Heywood, Middleton and Rochdale Primary Care Trust (HMR PCT) Heywood, Middleton and Rochdale PCT are seeking to establish a community based footcare service that delivers high quality patient centred care for people living with diabetes in the borough of Rochdale. The key objectives of the service are; To work collaboratively with local GP practices Provide direct patient care in the foot aspects of diabetes care Provide information, advice and education to people with diabetes and their carers Provide individual management plans to people living with diabetes Help improve quality of life for people living with diabetes It is anticipated that the contract will commence in June 2009. Interested parties are initially required to view the memorandum of information (MOI) and register their interest in this procurement. Please ensure you complete the technical questionnaire in order to view all tender documentation will be made available to registered suppliers on Monday the 2nd of March
