Community Service for COPD

Expressions of interest are being invited by Greater Manchester Commissioning Business Service (CBS) on behalf of Heywood, Middleton and Rochdale Primary Care Trust (HMR PCT) Heywood, Middleton and Rochdale PCT are seeking to establish a community based COPD service that delivers high quality care where care is provided in or closest to the patients home in primary or community settings. The key objectives of the service are; To work collaboratively with local GP practices in the identification and proactive management of people living with COPD To identify anticipated local prevalence of COPD Provide direct patient care Manage patients in their own homes or community settings Provide early discharge support Provide pulmonary rehabilitation programmes Develop agreed management plans with COPD patients Promote self management to patients and their carers through provision of education and information It is anticipated that the contract will commence in June 2009. Interested parties are initially required to view the memorandum of information (MOI) and register their interest in this procurement All tender documentation will be made available to registered suppliers who complete the EOI and technical questionnaire on Monday the 2nd of March.
