Locality Homecare & Support Service Framework - including Locality Plus

Provision of the Locality Based Home Care and Support Services for people aged 18 and over who are eligible for Adult Care Services living in Suffolk and NHS Continuing Healthcare (CHC) who are registered with a GP Practice in the Ipswich and East Suffolk Clinical Commissioning Group (IESCCG) and West Suffolk Clinical Commissioning Group (WSCCG) and the Great Yarmouth & Waveney Clinical Commissioning Group. (referred to as the CCG’s). Also included is Locality Plus which is live in, waking night, sleep in and block hours for people aged 18 and over who are eligible for Adult Care Services living in Suffolk and NHS Continuing Healthcare (CHC) who are registered with a GP Practice in the Ipswich and East Suffolk Clinical Commissioning Group (IESCCG) and West Suffolk Clinical Commissioning Group (WSCCG) and the Great Yarmouth & Waveney Clinical Commissioning Group.
